POSITION SUMMARY

The Day Habilitation Specialist is responsible for the direct provision of community and site-based Day Habilitation services to individuals with developmental disabilities.

ESSENTIAL RESPONSIBILITIES

  • Under the guidance and direction of the Manager of Waiver Services and through direct communication and collaboration with team of co-workers, develop, plan, and implement daily activities that support person-centered goals which help to foster community access, independent living skills through activities such as volunteerism and planned daily activities.

  • Prepare and submit a weekly plan of activities to the Manager of Waiver Services, prior to the commencement of each service week. Keep the Manager and co-workers advised of any changes in the plans.

  • Through direct communication with the Manager of Waiver Services, remain knowledgeable of each participant’s Habilitation Plan, goals, safeguards, and support needs and structure and plan daily activities such that they address skill acquisition and personal choice as stated in their valued outcomes. Instruct participants in identified skill deficits.

  • Responsible for the safe transporting of clients to and from daily planned activities.

  • Supervise individuals and monitor whereabouts at all times while in the community. Model appropriate behaviors using the environment to demonstrate and instruct on safety and appropriate behaviors. Remain knowledgeable of policy and procedures in the event of an emergency while in the community.

  • Act as a representative of Gateway Hudson Valley, Inc. when dealing with individuals and all community resources.

  • Complete all assigned paperwork, documenting participant’s progress and daily activities within specified timeframes.

  • Act as a crisis intervener when necessary.

  • Decorate the Day Habilitation room creating a new theme for each month.

  • Develop and implement art projects that are interesting and educational and ready by 9 a.m.

  • Provide back-up coverage for the Health Office as assigned.

    Health, Safety, & Compliance

  • Maintain a work environment in a clean, organized, and safe manner. Conduct daily inspections of work areas to look for unsafe conditions

  • Use all required safety devices and personal protective equipment.

  • Ensure all accidents are reported to supervisor and Human Resources.

  • Be familiar with the Health & Safety Program and abide by all safety and health rules, work practices, and regulations.

  • Maintain a clean and neat appearance consistent with agency standards and act as a role model

  • Attend all required meetings and trainings

  • Participate in training programs for professional growth as scheduled and approved

  • Responsible for understanding and adhering to Gateway Hudson Valley’s Corporate Compliance Policy and Standards of Conduct as well as all applicable program rules, regulations and policies. Keep abreast of any changes or modifications to policies or regulations. Ensure that all staff are knowledgeable of applicable regulations, laws, and policies

    REQUIRED QUALIFICATIONS

    Education/Experience:

  • Associate’s Degree in human services related field and 1 year experience working with individuals with developmental disabilities.

  • High School Diploma with two years of related experience accepted.

    Licenses / Certifications:

  • CPR/First Aid

  • Medication Administration (AMAP)

  • SCIP-r

    WORKING CONDITIONS

  • Must be able to Sit or stand for 8 hours/day

  • Must be able to bend down or kneel occasionally (1-2 times/week)

  • Must be able to use hands, wrists, and fingers with full range of motion

  • Must be able to perform CPR in emergency situation and for regular recertification.

  • May occasionally be required to lift no more than 50 pounds (1-2 times per month)

    REGULATORY CLEARANCES REQUIRED:

  • Must have a valid NYS Driver’s License and maintain a driving record which meets the insurable criteria of the Agency’s automobile insurance carrier

  • Driver’s License Abstract

  • Must have a favorable result from all state, federal, and agency mandated background checks

  • New York State Justice Center Fingerprinting Criminal Background Check

  • OWPDD MHL Abuse/Neglect History Check

  • Statewide Central Registry Check
