Job Summary and Qualifications
The Transplant Social Worker is responsible for assessing the psychological, social, cultural and/or financial needs that impact transplant patients and their families. Has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs.
What you will do in this role:
Knowledgeable in ESRD/transplantation Medicare guidelines
Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources
Serves as a transplant social worker resource for hospital case management working with transplant patients
Knowledge of payer source implications on plan of care and resources to maximize patient’s successful outcome
Provides intensive counseling services and direction for complex social and emotional problems of patients and their families
Identifies and understands the emotional factors underlying a patient’s illness
Performs a variety of services, such as psychosocial assessment pre-transplant for recipients & potential living donors
Advising on social problems, arranging for discharge (discharge planning) or postoperative care at home or in institutions, assists with evaluation for financial assistance to patients or families through MOKP, drug assistance, or other means post-transplant, and alleviation of anxieties or fears concerning permanent disabilities, or uncertainty about the future
Collaborates with Transplant Team to provide post-transplant support group meetings and newsletters
The Transplant Case Manager will work with all members of the team in accordance with the Mission and Values
What qualifications you will need:
Missouri State License required
Minimum of 3 Years of hospital Social Work experience is preferred
Excellent communications, organizational and documentation skills required
Must be able to work independently and with a variety of health care disciplines
Able to remove barriers for patient care and coordinate transition of patient into community care
Must have excellent written, verbal and interpersonal communication skills
Must be knowledgeable of State and JCAHO standards as well as ESRD/transplantation Medical guidelines
Must be able to provide crisis intervention and utilize system resources/referrals
Working knowledge of managed care and Medical health plans as well as all applicable federal and state regulations
