We are proud to offer you the opportunity to pursue your passion as a Physical Therapist, partnering on our Hospital in Your Home Program with a prestigious local hospital in our community and our Workman's Compensation medical team! The Hospital in Your Home experience is designed to provide hospital-level care in a patient’s home environment. This allows for deeper engagement between the clinical team and the patients, a consistent care team through phases of care, addresses goals of care and underlying social determinants of health care and designed and centered around the needs and wants of the patient. This is an experience we strongly believe in, and one that aligns with our own mission, vision, and values. While being admitted to their home, a patient may expect visits from Registered Nurses, Caregivers, Physical Therapists, Occupational Therapists, Dietary Specialists, Meal delivery services, Mental Health Specialists, Phlebotomists, Sonographers, Paramedics, Nurse Practitioners, and other healthcare providers. We are looking for a team of Physical Therapists (PT) who equally support this experience and are enthusiastic about replicating the capabilities of a brick-and-mortar hospital with in-home services. Our Physical Therapy team also assists in evaluating and developing care plans for our patients who have experienced an injury while on the job. Our therapy team follows these patients through the phases of care and updates goals and therapy plans/treatments as needed until goals have been accomplished. WE ARE NOT MEDICARE OR MEDICAID CERTIFIED - you will not use Oasis! If it's medically necessary for your patient to be seen 3-5x weekly, recommend it! We will work to approve the number of visits through their insurance provider.  Our therapists are typically working 5 - 15 visits per week! Physical Therapist PT) Responsibilities: * Evaluations patient's need for ongoing therapy * Provides physician-prescribed therapy * Improves or minimizes residual physical disabilities of the patient * Returns the individual to optimum and productive level within the patient’s capabilities * Participates with all other Hospital in Your Home personnel in patient care planning * Performs all skilled procedures as ordered by physician * Consult with physicians regarding change in treatment via telecommunication * Prepares clinical and progress notes and turns notes in within 1 hour of completed visit to BrightStar Care office Physical Therapist PT) Requirements: * Graduate from an accredited therapy program and pass the National Board exam. * Currently licensed in the state of Missouri * Ability to exercise initiative and independent judgment * Reliable car and auto insurance * Valid Driver's License * Current CPR Certification (BLS- Red Cross or American Heart Association) we offer a class at a reduced cost if you need your BLS * Negative TB test within last year * Clean background and criminal record * COVID Vaccinated * Understands regulatory compliance with state and federal regulations * Adhere to HIPAA and maintain client confidentiality This position requires that you provide one weekend per month with availability for Hospital in Your Home or Work Comp visits.
