Description Community Habilitation Specialist

Full Time, Part Time and Per diem positions available. $16.32/hour SUMMARY OF DUTIES

As a Community Habilitation Specialist, you will work directly with individuals with developmental/learning disabilities, providing services to the people we support so they can live independently and thrive in their communities. This could include, but is not limited to: * Community integration * Community travel and transportation * Money management and budgeting * Appropriate socialization/interactions * Healthy lifestyle skills/exercise * Medical appointments and understanding doctor's orders * Additional skills needed to enable the individual to access the community and remain as independent as possible * Other instruction, services and supports as indicated by your supervisor.

SKILLS & ABILITIES * Possess good time management skills * Ability and desire to work as part of an interdisciplinary team * Ability to work 1-1 with an individual receiving services * Moderate level of written and oral communication skills * High level of interpersonal skills * Knowledge of behavioral techniques and developmental disabilities to assist in the care of individuals receiving services * Knowledge of computer and ability to utilize various software programs * Successful completion of First Aid, CPR, and Defensive Driving upon hire is required.

EDUCATION & TRAINING Preferred: One year pertinent experience in health and human service related field and experience with individuals with disabilities.

Minimum: High school diploma or equivalent

Valid New York State Driver's License in good standing and reliable vehicle, mileage reimbursement and use of personal vehicle for transporting individuals receiving services.
