Summary

Interact and teach identified patient populations about total disease management. Assist in throughput of patient utilizing established multidisciplinary teams. Assist physicians and staff in the quality management of specific disease processes. Conduct educational sessions for staff and physicians as needed and participate in community awareness programs. Assist in data abstraction, analysis and reporting and in the development of action plans to correct variances in quality care. Perform all other duties as assigned.

Responsibilities

  • "Provides education on disease management for an identified patient population. This includes interaction with the patient, family, staff and physicians. 20 %

  • Assist in the throughput of the patient and family through the continuum of care for their disease process. This is to include interaction with the multidisciplinary team (physicians and all hospital departments). 20 %

  • Actively participate in performance improvement teams and task force teams to develop, implement and monitor action plans for improvement in the care of the patient. 20 %

  • Actively participate in the procurement of required quality data during the patient's hospital stay. 20 %

Requirements, Preferences and Experience

Education

Minimum: Graduate Registered Nurse Program-Accredited Required.

Experience

Preferred: Healthcare/Medical-Nursing-Cardiovascular Preferred. Healthcare/Medical - Nursing - Neurology Preferred.

Minimum: 3 years Healthcare/Medical-Nursing General Required.

Licensure, Registration, Certification

Preferred: Advanced Cardiac Life Support (ACLS) Certification Preferred.

Minimum: License/Certification/Registries (valid for the State of MS): Registered Nurse (RN) - Unrestricted Required. Basic Life Support (CPR) Certification within 14Days of Employment Required.

Special Skills

Preferred: Efficient in the use of basic computer software (Microsoft Office) preferred.

Minimum: Excellent communication and people relationship skills. Must be able to use critical thinking skills to problem solve.
