Henry Ford Health System Registered Nurse- In Home Hospice - Jackson - Part Time in Jackson, Michigan

GENERAL SUMMARY

The Registered Nurse - Hospice provides comprehensive, convenient, and high-quality Hospice services to meet the physical, emotional, spiritual, and social needs of adult and geriatric patients with limited life expectancy.

PRINCIPLE DUTIES AND RESPONSIBILITIES

  1. Coordinates patient/family care, across disciplines and other members of the IDT to ensure the physical, emotional, spiritual and social needs of the patient/family are met. This will be accomplished with quality and efficiency in mind.

  2. Provides direct nursing care in the place of residence to adult and geriatric patient populations ensuring implementation of nursing processes and professional standards of care.

  3. Maintains a safe and risk-free environment for patients, their families/significant others, and other staff.

  4. Participates in agency and system-wide continuous quality improvement activities to improve performance and patient outcomes.

  5. Recognizes the importance of building partnerships with all members of the health care team and exhibits behavior that supports the Standards of Service Excellence, including teamwork, professionalism, and respect for diversity.

  6. Demonstrates personal accountability for own continuous growth and development and seeks out appropriate learning opportunities to enhance competency and improve performance.

  7. Creates a caring experience for both internal and external customers; is courteous and respectful, anticipates and responds to customer needs; and give high priority to customer satisfaction.

  8. Collaborates with supervisor to develop and implement policies, procedures, quality improvement activities, reporting, training, and education to ensure organization wide compliance with legal, ethical, contractual, and/or regulatory standards (i.e., JCAHO, HCFA, OIG, Corporate Compliance, MIOSHA, etc.)

  9. Recognizes the importance of orienting members of the health care team and exhibits behaviors that promote a positive learning experience for the orientee.

EDUCATION/EXPERIENCE REQUIRED

  • Minimum Education -Associate’s degree in Nursing (ADN)

  • Minimum Work Experience -Minimum of four years nursing experience in a Hospice environment

  • Required Licenses/Certifications

  • 1.Current Michigan Registered Nurse license

  • 2.Basic Life Support (BLS) certification.

  • 3.Valid Michigan Driver’s license.

  • 4.Dependable automobile insured according to o State of Michigan requirements. 

Required Skills, Knowledge, and Abilities

    1. Effective written, verbal and interpersonal communication skills via phone and face-to-face.
    1. Ability to problem-solve and excellent critical thinking skills.

Preferred Qualifications

  • 1.Bachelor’s Degree in Nursing (BSN).

  • 2.Previous nursing experience in Hospice or Home Health setting.
