Make a Difference

The Medication History Technician will obtain and document medication histories in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards.

Your Exceptional Skills and Qualifications

· Performs thorough interviews with patients and family members with appropriate professional language.

· Conducts telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication history with appropriate telephone etiquette.

· Interprets and enters “Prior to Admission” medication history into the electronic medical record in an accurate and timely manner.

· Coordinates responses and resolutions to issues with appropriate internal and external parties.

· Regularly and appropriately communicates with supervising pharmacists for any questions and additional work assignments.

· Accurately tracks interventions and productivity using department’s process.

· Maintains proficiency with departmental information system technology and documentation systems.

· Maintains contemporary professional knowledge base.

· May represents department on various hospital committees.

· Participates in evaluation of quality service, development, and implementation of improvement.

· Communicates with other hospital employees and departments as necessary.

· High School Diploma or GED (Required)

· One of the following is required CertIfications/Licensures: (Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA), Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as Pharmacy Intern (IPLA) you do not need to be nationally certified with PTCB or NHA.)

· within two (2) years from hire CertIfications/Licensures: (Pharmacy Technician Certification Board (PTCB) National Certification or National Healthcareer Association (NHA) (ExCPT))
