Make a Difference

The Manager of Case Management provides onsite leadership to ensure daily department operations function effectively and efficiently to meet the needs of all customer and regulatory groups. You will act as a consultant to ensure the severity of illness and intensity of services interval criteria evaluations are completed on all patients as designated by Case Management goals. You will ensure that case managers assess, plan, and develop plans for patients based on recognized standards of practice for case management. You will support a culture of continuous quality improvement.

Exceptional Skills and Qualifications

Applicants for this position should be able to demonstrate competency as a Case Manager, exhibit team-building skills, and effect change by assuming leadership roles in interdisciplinary teams.

  • A Master's Degree in Social Work from an accredited school of social work is required.

  • Three (3) years of experience in a hospital setting as a Social Worker is required.

  • Two (2) years of leadership experience is preferred.

  • Case Management experience is preferred, but not required.

  • Current LSW or LCSW in the State of Indiana is required for social work candidates.

  • Understands the relationship of individual patients, units, and/or departments to the network, to facilitate achievement of optimal goals with those relationships.
