Where you fit in

The Case Manager MSW or RN is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. Responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family. Provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and management and discharge planning. Has accountability for the care, coordination, and discharge planning of inpatient and observation services.

Your exceptional qualifications

  • MSW - Master’s Degree in Social Work required. Three years of recent clinical experience preferred. Currently licensed by the State of Indiana as a Social Worker required (or contingent upon obtaining within 90 days of employment).

  • RN – Graduate of National League for Nursing, Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing. Licensed as a Registered Nurse (RN) in the State of Indiana required. Two to three years of recent clinical experience or three years recent hospital case management experience.

  • Certified Case Manager (CCM) or ACM preferred.
