GENERAL RESPONSIBILITIES: Provide nursing, MAT, and special project duties as assigned by physicians, mid-level providers and department/project supervisors.

ESSENTIAL JOB FUNCTIONS:

  1. Maintain and manage schedules related to job duties.
  2. Attend training as warranted and directed per needs of department/special projects.
  3. Able to work effectively under pressure.
  4. Assists in tracking MAT clients and assuring adherence to MAT policies and procedures.
  5. Adheres to contact standards required for each assigned EBP and manages assigned caseload effectively.
  6. Communicates with all programs/departments involved in care of MAT clients.
  7. Attends and demonstrates competency in all assigned EBPs (MAT, ACT, Crisis Services).
  8. Expect travel between all FCMHC locations.
  9. Willingness to provide mobile services in conjunction with community based programs.
  10. Exhibits sound judgement in crisis and routine situations.
  11. Communicate and work effectively with outside agencies, including physicians, nurses, public health, pharmacies, and other community resources.
  12. Maintain positive, cooperative professional relationships with all levels of staff in the medical services department and across the agency.
  13. Maintain all clinical documentation in an accurate and timely fashion in compliance with all Federal, State, and Four County guidelines.
  14. Administers and monitors medications as directed by provider.
  15. Ability to provide medical assessments, refills and injections, caseload and project management, and charting in an electronic health record, all under the direction of Psychiatrist, PA, APRN and/or Director of said programs.
  16. Provide consultation to community caretakers and provide in-service and groups as direct by Center.
  17. Exhibit and promote decisions/behaviors consistent with the Centers established Vision, Mission, and Values.

OTHER JOB FUNCTIONS:

  • Any deviations to the work schedule and all overtime must be approved in advance.
  • Other duties as assigned by Supervisor.
