Description:

Manager, Acute Care Flow Operations opportunity with oversight of Trinity Health Livingston (Howell, MI) and Chelsea Hospital (Chelsea, MI). These departments represent 30+ staff members in ED case management, In-patient case management, In-patient social work, In-patient Rehab case management and social work and case management assistant roles.

This is a hybrid position requiring a presence at Trinity Health Livingston (Howell, MI) and Chelsea Hospital while offering remote work opportunities, as needed.

The Manager Acute Care Flow Operations provides leadership and operational direction in the areas of business issue identification and analysis, business process redesign, effective organizational structure and restructuring, best practice methodologies, and identification of strategic and financial opportunities/risks within the Department of Acute Care Flow and Care Management. The Manager will supervise some staff in the department and will be involved in coordination, planning, logistics, data analysis, technology, patient flow modeling, scheduling and day-to-day operational activities to support optimal patient flow in order to help meet hospital goals.

They Collaborate as change leader in translating issues, goals, and objectives into practical and integrated solutions and service that ensure optimum clinical quality and business performance. Conducts work from both a strategic perspective and a tactical perspective. Position functions at multiple levels in the organization, including single department, interdepartmental and hospital wide. Acute Care Flow is a complex set of teams and processes with vital organizational outcomes - length of stay, readmissions, denials.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Manages the overall flow and movement of patients through the hospital and from all entry points, ensuring patients are placed in the most safe and efficient manner.

  • Oversee and manage staff, which includes responsibility for the hiring, supervision, development and evaluation of direct reports.

  • Represents Trinity Health Livingston and Chelsea Hospital in discussions with Extended Care Facilities and other health care organizations to build collaborative work processes, which facilitate coordination of care for all patient populations.

  • Secures resources, defines roles and responsibilities, sets direction, aligns and coordinates initiatives related to enhanced operational performance or sustaining the ministry goals. May lead, direct and/or participate in the vendor selection process including identification, evaluation and selection, the development of contracts and agreements, and contract compliance for tools and services related to perfo1mance improvement initiatives

  • Develop, maintain, and evaluates discharge planning processes and procedures for designated patient populations.

  • Organization operating status reviews. Provides assistance in both the analysis and implementation of initiatives specific to patient/customer service, employee engagement, quality and community benefit ministry. Provides staff support to entities and committees as required.

  • Prepares period written reports and makes presentations to hospital leadership, summarizing activities, accomplishments, and future recommendations for Acute Care Flow and Care Management.

  • Develops, establishes and supports planning and continuous improvement initiatives for patient throughput.

  • Develops and maintains policies needed to maintain effective operations within Acute Care Flow and Care Management

  • Ensures compliance with The Joint Commission and Center for Medicare & Medicaid Services (CMS)

  • Uses predictive modeling and analytical tools to drive improvements in LOS (length of stay), as well as improve the hospitals forecasting ability to react to capacity escalations

  • Provides appropriate, timely interventions at key points to negotiate, procure and coordinate resources in order to achieve clinical, financial, and quality outcomes

  • System administrator for various systems and tools. Utilizes various analytical tools such as benchmarking, simulation, workflow queuing models, and sampling in conjunction with effective interpersonal skills to analyze operational issues. Determines and recommends improvement alternatives. Works in conjunction with staff, consultants, management and administration to design project plans, conduct studies, make recommendations and develop implementation plans for operations improvement that enhance revenue, productivity, resource utilization, clinical quality, and/or customer service.

  • Acts a liaison with the Acute Care Flow and Care Management team and other members of health care team (EVS, transport, physicians, nurses, and other ancillary departments) in the overall coordination of optimal patient flow during a patient's hospital stay

  • Assists the team in understanding the viability and use of appropriate operations performance enhancement methods, resources and tools. Oversees the development of policies, standards, protocols and metrics that ensure resources or services are tailored, responsive, tracked, enhanced, deliver high value and educate executives and their associates how to improve performance and solve business problems. Directs leaders to formal training and coaching programs, as needed, to educate on cost-benefit analyses, facility planning, and other operational improvement methods/tools.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

  • Bachelor of Science degree in Management, Engineering, Health Care Administration and/or BSN or other clinically related degree. Master's degree preferred.

  • Experience: Health care experience preferred. Minimum of six months experience, with increasing levels of responsibility and titled leadership, or an equivalent combination of education and related experience.

REQUIRED SKILLS AND ABILITIES

  • Possesses working knowledge of standard operational performance methods such as Six Sigma, Lean and Collaboration Team facilitation.

  • Demonstrated, progressive leadership responsibilities including project management, program development, data management.

  • Possesses expert skills in managing productivity, demonstrating proficiency in the use of various tools and methodologies and successfully coaching all levels of the organization to manage established targets.

  • Comprehensive knowledge of health care operations preferred.
