Job Description

Your responsibilities include providing assessment, counseling, clinical project management support, clinical work groups support, and information and referral to patients and their families. We are looking for someone sensitive and perceptive, with a strong sense of community care and social service, who shares our commitment to creating a healthier Hawai‘i.

Minimum Qualifications: Master's Degree in Social Work. Current Hawai‘i Social Work license. Two (2) years of social work experience. Experience working within a collaborative interdisciplinary team. Current driver's license and valid Hawaii auto insurance with short notice access to an automobile to conduct site visits, as required. Current BLS card.

Preferred Qualifications: Current Hawai‘i Licensed Clinical Social Work (LCSW) license. Experience in care coordination for patients. Previous social work experience in patient population covered by applicable program. Experience working in clinical projects with interdisciplinary clinical workgroups. Working knowledge of EMR (Electronic Medical Record).
