As the Care Management Services Registered Nurse, you will provide care management and coordination for patients enrolled in the Care Management Program in the Primary Care setting in partnership with the primary care practice health team under Hawai‘i Health Partners. You will serve in an expanded health care role to collaborate with specialists, members of the health care team and patients/families/caregivers to ensure the delivery of quality, efficient and cost effective health care services. We are looking for someone dynamic and dedicated, with great organizational and communication skills and a commitment to creating a healthier Hawai‘i.

Minimum Qualifications: Bachelor’s Degree in Nursing or related field. Current Hawaii Registered Nurse (RN) license. Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card. Current driver’s license and valid Hawaii auto insurance with short notice access to an automobile to conduct site visits, as required. One (1) year of clinical experience in a Primary Care setting and experience working within a collaborative interdisciplinary team.

Preferred Qualifications: Certification in Case Management. Two (2) years of clinic experience in a Primary Care setting. Care management and coordination experience with focus on continuous quality improvement. Working knowledge of EMR (Electronic Medical Record).
