The New Jewish Home Fee For Service Field Nurse - Manhattan and Bronx $70-$125 per visit in Home Assistance Personnel Inc. - Bronx, New York

The New Jewish Home has a long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible.

We are currently seeking a Fee for Service Nurse to join our team who will provide home care services in the Manhattan and Bronx areas!

The qualified candidate will:

  • Provide in-home care management for complex chronically ill clients

  • Provide a full range of nursing care duties in the client’s home including assessment, education and medication management

  • Coordinate client’s care with care team and create care plan for client’s care

  • Document care in EMR in a timely manner

    Minimum Requirements:

  • A valid NYS RN license

  • A minimum of one (1) year home care experience as an RN with a CHHA or LHCSA

  • A minimum of one (1) year in an acute care or hospital environment
