The Credentialing Associate is responsible for the operational aspects of the medical staff appointment and reappointment process ensuring compliance with all regulatory agency requirements. Assists the Manager of the Medical Staff Office in other duties as required. Must be able to multitask, work independently and have excellent communication and prioritization skills.

Responsibilities

  • Accurate Data: Ensure completeness and accuracy of information entered into the database. Accurate and complete data should be entered into all required fields. Updates the database with all changes.

  • Prepares appointment and reappointment letters. Prepares letters in time for signature the day of Board of Trustees action. Assures that all practitioners are informed of actions in a timely manner and tracks return of acceptance letters.

  • Appointment Status: Provide constant status follow-up of all pending reappointment and appointment applications. Communicates with the practitioner for all applications that are not received or are incomplete.

  • Reappointment Packets: Responsible for activities relating to the production and distribution of the reappointment packets for the Medical Staff and Health Professional Affiliates Utilizes software to track application packets that need to be sent. Prepares packets and associated enclosures. Assures that all packets are complete and accurate before sending. Ensures that all packets are sent as per the bylaws or as directed.

  • Reappointment Report: Prepare reappointment report in the approved format. Prepares the report and has it ready for distribution with the packets according to the policy and established timelines.

  • Credentials Files: Create and maintain credentials files on physicians. Files are maintained with accurate, up to date information easily available for retrieval. Information is placed in the files correctly as per policy.

  • Initial Appointment Credentialing: Assists the Manager with initial appointment credentialing. Receives requests for application packets and forwards the packets upon approval. Receives the information from the applicant and the CVO and places it in the file.

  • Negative Issues/Actions Found: Notifies supervisor of any negative issues/actions found. Identifies 'Red Flags' and immediately notifies the supervisor of any negative information found. Requests follow up documentation as necessary.

  • Practitioner Profiles: Prepares practitioner profiles and obtains data from other departments as required and prepares files for review. Compiles data and organizes for department chairman review.

  • Other duties and/or projects as assigned.

  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.

  • Current or prior credentialing experience in a hospital, managed care or physician office setting.

  • Excellent written and verbal communication skills.

  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Licenses and Certifications Required:

  • CPCS or CPMSM or ability to obtain certification within 2 years of hire.
