Volunteers of America Northern Rockies SSVF Healthcare Navigator in Helena, Montana

Summary/ObjectiveThe Supportive Services for Veteran Families (SSVF) Health Care Navigator will provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. SSVF health care navigators offer case management, care coordination, health education, and administrative duties. SSVF Health Care Navigators will work closely with the Veteran's primary care provider and the Veteran's assigned interdisciplinary treatment team members.Essential FunctionsConduct assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others to understand the Veteran's situation, potential barriers to care, the causes, and the impact of such restrictions on the Veteran's ability to access and maintain health care services.Work closely with Veterans in Emergency Housing (EHA) to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision-making of the Veteran's care.Serve as a resource for education and support for Veterans and families and helps identify appropriate and credible resources and support tailored to the needs and desires of the Veteran.Provide case management in conjunction with the housing Service Coordinator and care coordination across episodes of careacts as a health coach by proactively supporting the Veteran to optimize treatment interventions and outcomes.Coordinates and links Veterans and caregivers to supportive services and will serve as a subject matter expert on community resources related to the needs of the Veteran.Provide ongoing education support and assist in identifying VA and community resources to promote self-care as needed to the Veteran and family members.Regularly consults with other team members and appropriately assesses and addresses the needs of the Veteran.Provide traditional SSVF case management  services as directed by the Program ManagerDevelop relationships with community leaders, VA staff, and other referral networks.Other duties as assigned.CompetenciesCrisis managementAbility to work with multiple staff, clients, and agencies.Proficiency with Microsoft Office Software: Outlook, Word, ExcelAble to meet tight deadlines under pressure.Organization skillsAbility to work both independently and as part of a team.Practical oral and written communication skillsContinuous demonstration of behaviors aligned with a posture of servant leadership and our cultural values.Work EnvironmentThis job operates in a residential facility environment.
