Are you ready to be at the forefront of making a positive impact on the mental health and well-being of youth in our community? High Plains Mental Health Center is seeking a dedicated and passionate individual to join our Community Based Services (CBS) team as a School Liaison.

Your Mission:

As School Liaisons at High Plains Mental Health Center, youll be a champion for the mental health and well-being of youth in our community. You will play a crucial role in coordinating with area school systems to identify, refer, and monitor youth in need of mental health services. Your dedication to building strong partnerships with school systems, empowering families, and supporting treatment teams will ensure timely access to mental health services for youth experiencing severe emotional disturbance. Through collaboration, education, and advocacy, you will break down barriers and facilitate the highest level of functioning for youth in the school, home, and community settings. Together, we are all committed to making a positive impact on the lives of those we serve, fostering resilience, and promoting healthy futures for our youth.

Major Responsibilities:

Building and maintaining positive relationships with school systems

Coordinating with schools to ensure early identification of youth in need of mental health services

Providing training and support for key school staff regarding agency referral processes

Facilitating timely referrals of identified youth through coordination with schools and clinical supervisors

Engaging with families to educate them about available services and options

Collaborating with schools, families, and treatment teams to monitor youth progress and ensure appropriate service delivery

Completing accurate and timely documentation, tracking, and reporting

Delivering CBS/Waiver services as needed, including targeted case management and psychosocial rehabilitation

Developing and maintaining professional relationships with colleagues

Accepting supervision and utilizing feedback for continuous improvement

Qualifications:

The ideal candidate for the School Liaison position at High Plains Mental Health Center will possess a Bachelor\'s degree (BA/BS) or equivalent work experience in human services, coupled with a proven track record of building positive relationships with school systems and community partners. Experience working with youth experiencing severe emotional disturbance and their families is essential, as is proficiency in technology use, including various software applications.
