Van Dyk Health Care at Park Place- Assisted Living in Hawthorne, NJ is accepting applications for LPN- Licensed Practical Nurse * Per Diem *

Pay Rate = $30-$34 per hour

Shifts:

6:00am-2:00pm / 2:00pm -10:00pm / 10:00pm - 6:00am

  • Must be able to work some weekends *

Duties & Responsibilities

  • Implement and maintain established nursing objectives and standards.

  • Make independent decisions concerning nursing care.

  • Notify resident's physician and next of kin/sponsor, when there is a change in the resident's condition.

  • Maintain confidentiality of all resident care information. Supervise the self-administration of medications as ordered by the physician.

  • Receive telephone orders from physicians and record on telephone order form to be signed and returned within 48 hours.

  • Transcribe physicians to pharmacy order sheets, lab requests forms and MAR (medication Administration Record)

  • Perform routine charting duties in accordance with established procedures.

  • Sign and date all entries in resident's medical file.

  • Chart all accident/incidents involving residents and follow established procedures.

  • Perform day-to-day functions of the nursing activities in accordance with current rules, regulations and guidelines that govern this facility.

  • Perform administrative duties such as completing medical forms, reports, studies, charting, etc., as necessary.

  • Participate in surveys (inspections) made by authorized government agencies.
