We are hiring a Hospice Aide to join our dynamic care team at Hospice Services of Massachusetts!

This position is located in the Cape area, covering Wareham, Rockland, Braintree, Weymouth, Hanover, and other areas.

As a member of our hospice team, you carry out duties assigned by the case manager to increase patient comfort, to maintain good personal hygiene, and a safe environment for the patients.

Education and Experience:

  • Pass the Home Health Aide competency exam.

Duties and Responsibilities:

  • Adheres to the written home health aide plan of care as prepared by the RN Case Manager.

  • Promptly reports changes in the patient’s condition to the RN Case Manager.

  • Assists the patient with all aspects of personal care, ambulation and positioning as required.

  • Prepares meals and assists with feeding as required.

  • Performs household services which are necessary for the health and safety of the patient and family.

  • Provides assistance with bedpans and urinals as needed.

  • Provides guidance to the caregiver regarding personal care of the patient.

  • Attends and participates in weekly team meetings, AM report, inservices, and agency meetings and other related activities when appropriate.

  • Understands and adheres to Hospice Services of Massachusetts’s policies and procedures.

  • Follows the guidelines for universal precautions when providing patient care.

  • Maintains records of services performed and of apparent condition of patient, using documentation guidelines and standards set forth by Hospice Services of Massachusetts.

  • Submits legible and accurate documentation of all services provided in a timely manner in accordance with home health aide documentation requirements.

  • Accept other assignments as appropriate.

Specific Requirements:

  • The ability to read, write, speak, and understand the English language.

  • Must possess the ability to make independent decisions when circumstances warrant such actions.

  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.

  • Must be able to physically perform personal care on clients.

  • Must be able to handle confidential patient information and medical records.

  • Must have reliable transportation and a driver’s license.
