In this role, *the | | | Veteran Healthcare Navigator will | | | serve veteran families from | | | Jackson to the MS Gulf Coast, | | | with families from the Pine Belt | | | Mental Health service area being | | | the primary focus. | | | * | | | | | | | | | | | | | | | For 50 years, Pine Belt Mental | | | Healthcare Resources has served | | | individuals throughout the Pine | | | Belt. Through an extensive | | | continuum of care, we strive to | | | enable adults with serious mental | | | illnesses, children with serious | | | emotional disturbances, and | | | individuals with an addiction or | | | developmental disability to live, | | | work, learn, and participate | | | fully in their communities. Our | | | eighteen county service area | | | includes Amite, Franklin, | | | Forrest, Hancock, Harrison, | | | Jones, Lamar, Lawrence, | | | Covington, Greene, Jeff Davis, | | | Marion, Pearl River, Perry, Pike, | | | Stone, Walthall, Wayne Counties. | | | | | | | | +-----------------------------------+-----------------------------------+ | | | | Requirements: | | +-----------------------------------+-----------------------------------+ | - Master's degree in | | | behavioral healthcare or | | | related field preferred | | | - LCSW or equivalent background | | | preferred | | | - Must have reliable | | | transportation to provide | | | community services | | | - Must have a valid driver's | | | license with a satisfactory | | | driving record | | | - Excellent communications | | | skills required | | | - Independent work ability | | | - Veterans encouraged to apply | | +-----------------------------------+-----------------------------------+ | | | | Responsibilities: | | +-----------------------------------+-----------------------------------+ | - Maintains and updates contact | | | list of VA and other | | | community programs. | | | - Maintains successful working | | | relationship between SSVF | | | program and providers. | | | - Identifies and attempts to | | | remove barriers to care for | | | veteran participants. | | | - Assists veterans in gaining | | | access to healthcare, | | | supporting healthcare plans | | | by identifying barriers to | | | care, and providing education | | | on wellness-related topics. | | +-----------------------------------+-----------------------------------+
