Job Summary Provides routine social services as a member of a multi-disciplinary team, including obtaining social histories, providing counseling and guidance, referrals to agencies that respond to the psycho-social needs of patients/clients and families. As appropriate identifies and refers, or seeks supervision on complex cases.
