UP Health System – Portage in Hancock, Michigan invites qualified Advanced Practice Providers to join our Internal Medicine clinic team. Nurse Practitioners and Physician Assistants to be considered.

Essential Functions: • Provide exemplary customer service to patients, visitors and coworkers. • Provide convenient, friendly patient care to all who come through our door seeking health related services. • Obtains patient history and performs assessment via observation, interview and examination. • Provide direct treatment and management of health conditions, including use of prescriptive authority and referral to other healthcare providers or community resources. • Documents patient care performed, assessments made, and orders given. • Oversees, directs and supports on site laboratory, x-rays, injections, and collections for direct access services, if applicable. • Provide counseling and education of patients and their families/caregivers concerning preventative health, treatment options and community resources. • Provide care in the context of promoting age- and culture-appropriate informed choices, shared decision making, and the right to self-determination. • Working environment subject to varying fast paced conditions during peak seasons. • Maintains clinical core competency.

Position Requirements: Master’s degree required Current MI Nurse Practitioner or Physician Assistant licensure required Annual influenza vaccination or declination required

Position Details: 1.0 FTE (full-time, 40 hours per week) Day shift, weekdays only 8 hour shift On-call rotation
