Contributes to the development, implementation, monitoring, and updating of Consumers’ individualized habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements. Other duties as assigned by management staff. Productivity requirement of 60% face to face contact weekly. Must be able to operate an Agency vehicle and maintain a driving record acceptable to Agency’s insurance provider.

    Bachelor’s Degree from an accredited college or university in oneof the helping professions, such as social work, counseling, psychiatric rehabilitation, or psychology. Bachelor’s Degree in a field other than a helping profession shall have a minimum of two years’ professional experience working in a community based behavioral health setting. Certified Psychiatric Rehabilitation Practitioner (CPRP) may be substituted for one year of experience.
