The Care Manager provides care management and stabilization to youth and families. Conducts follow-up contacts and assessments telephonically or in community. Maintains active list of youth and families. Reaches out to post intervention clients to perform follow-up. Answers 24-hour hotline, as scheduled. Updates Consumer log on a daily basis. Works in a team approach. Updates the CSA with clinical information. Works with Lead Response worker with additional duties such as attending trainings, conducting informational sessions for outside agencies, and other duties as needed.

Legacy operates the State-designated Children’s Mobile Response and Stabilization program for Burlington County. The program provides crisis intervention to families with a child or youth exhibiting emotional or behavioral disturbances that threaten to disrupt their current living arrangements. Trained response workers diffuse crises at the site of the crisis for up to 72 hours. They work with the family to develop an individualized crisis plan and coordinate in-home follow-up intervention services for up to eight weeks.

Children’s Mobile Response and Stabilization Services are available 24 hours a day, 365 days a year.

Education/Experience:Master’s Degree in a related field or Bachelor’s Degree in a behavioral health or related human services field such as social work, counseling or psychology, and have a minimum of one year of related experience.
