The Clinical Navigator --RN is responsible for coordinating activities involving patient populations through risk stratification activities requiring one on one support in coordinating care activities, supporting and assisting patients to navigate through the steps in obtaining their necessary and appropriate care. The Navigator works collaboratively with the primary care providers and care managers in support of their efforts to address patient care needs and opportunities. In order to effectively advocate for patient needs, the Navigator may meet with patients and perform/gather routine health screening data. The Navigator works with patients to ensure ongoing review of routine and ongoing care opportunities in an effort to meet established quality goals and metrics. Specialized Responsibilities: (This list may not include all of the duties assigned) * Under the direction of the Chief Quality Officer, works closely with data analysts to identify primary and secondary preventative care gaps and develops initiatives for specific patient populations. * Responsible for the development of programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. * Analyses risk stratification data to identify high and rising risk populations. * Effectively develops and manages patient navigator program and activities to address the needs of specific patient populations. * Uses an integrated approach to patient outcomes. Utilizes standards, guidelines, and protocols for determination of care needs and develops initiatives to meet established goals. * Incorporates data and information to continuously make recommendations to improve services and practice workflows to enhance outcomes. * Assist patients in understanding resources available including educating patients about appropriate screening and care opportunities. * Work with data analysts to assess the needs of specific populations and establish programs to meet identified...
