As part of the Adult Outpatient Program, you will work with an integrated team of professionals from both the mental health and substance use fields, who are cross-training in assessing and treating both voluntary and legally involved clients, via individual, group, case management, and couple's treatment. This team provides a solution-focused, cognitive behavioral, and/or motivational interview approach to bring about positive transformations in a shorter amount of time.

Duties Performed: Assist clients in achieving their goals as stated in their treatment plans or as indicated by appropriate brief interventions. Collaborate with therapists and other relevant staff to coordinate services, and address client needs and behaviors; prevent deterioration and/or hospitalization through consistent and adequate monitoring as indicated by program scope. Refer, coordinate, and assist clients to access a wide variety of services (e.g. vocational, medical, social security, housing, etc.) based on need and client desire; aid clients in application processes and provide supporting documentation as needed. Maintain close and cooperative relationships with community agencies, other public agencies, and private businesses (i.e. landlords, banks, etc.) in carrying out supportive services. May maintain contracts, leases, and other paperwork on behalf of clients. Facilitate client engagement in services and education. May teach approved curriculum for psychoeducational classes/groups with modification of teaching techniques to engage all participants in learning. Educate the client on behaviors to maintain daily life activities. Aid and guide clients with daily life activities (e.g. managing finances, medical treatment recommendations, physical and emotional self-care, etc.). Offer support, assistance, consultation, and education to relevant providers, family members/caregivers, and other involved parties when necessary to adequately support clients and minimize barriers. Ensure ervice engagement and delivery as necessary by adapting to client needs (i.e. visiting clients in various environments, conducting active outreach to clients, accompanying clients to appointments where clinically indicated, setting up medications in med-minders, etc.). Assist clients in improving their well-being and quality of life by coaching in behavioral skills, educate clients to make informed choices; develop coping skills of clients; help clients to problem solve and negotiate tasks of daily living. Monitor, evaluate, and record client progress concerning treatment goals. Use electronic systems to document process reports, case management notes, letters, faxes, and phone calls. May input, organize, and maintain detailed clinical performance data to comply with applicable government program

Education/Credentials: A Bachelor's Degree in Human Services field from an accredited college or university is required.
