Job Description:

Position Summary:

Responsibilities include: Psychosocial assessment, supportive counseling and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, med/surg population. Population groups may include adult, geriatric, pediatric, neonatal, pregnant and parenting teens on inpatient medical and surgical floors and on an outpatient basis as assigned.

Experience:

At least one year related experience. Must be able to triage crises and coordinate with collateral agencies. Experience with protective services, child welfare, long term care, crisis intervention, chronic illness, hospice care, substance use disorders and psychiatric illness.

Full time Hospital experience preferred MSW preferred will consider lcsw.

Education:

A candidate will have successfully completed a Master's degree in Social Work

Certificate/License:

Current Massachusetts License in Social Work
