The Impact You Can Make * The Care Manager (Acute Care) I (CM I) role must optimize and integrate the interface between clinical and financial systems supporting positive health outcomes. Responsible for the assessment, integration and facilitation of patient discharge plans focusing on the coordination of care for the medium to high risk patients across the healthcare continuum. Goal is for discharged patient to possess a “reasonably safe discharge plan”, engaging support systems if patient allows and are available. Ensures the delivery of cost effective, outcome-oriented and quality of care within an expected length of stay. Manage patient care to prevent fragmentation and duplication of services. Works in collaborative practice with the physician and other members of the health care team to meet patient specific and age related needs linking cost and quality to patient care. Education/Experience: * RN or Social Work degree preferred (MSW or BSW). * Human services degree with 2 years of direct care experience accepted. * Home care, hospital, or nursing home experience strongly preferred. Licenses/Certificates/Registrations: * Maintains current licensure and/or certification with appropriate professional affiliation. Salary Range The expected base rate for this Glens Falls, New York, United States-based position is $32.00 to $58.74 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
