CVS Health Senior Manager of Social Determinants of Health Product Management in Georgia

Job Description

This is a remote role and may sit anywhere in the US. However, travel is required - approximately 60%• Implement and scale the overall strategy to address SDoH for members across Medicaid enterprise • Operationalize SDoH product in local markets • Hire, train, and manage field staff in assigned markets • Identify opportunities to develop and pilot new SDoH initiatives• Gather input and maintain proactive communication across business units to coordinate SDoH function across the enterprise• Integrate SDoH solutions into population health, care management, quality, regulatory, and community outreach programs • Partner with data analytics team to identify evaluation metrics and evaluate outcomes • Oversee execution of community initiative funding supporting community gaps and member needs • Establish and build key relationships with community-based organizations to execute SDoH strategy• Organize community activation meetings to successfully execute key SDoH initiatives with community partners • Create and support Business Development initiatives to align with SDoH strategy

Pay Range

The typical pay range for this role is:

Minimum: 75,400

Maximum: 162,700

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Required Qualifications

• Bachelor’s Degree or higher in health administration, social services, public health, business administration, psychology or closely related field • Licensure or certification if applicable to degree • 10 + years’ experience with population health, care coordination, and/or community-based programs • 5 + years in a leadership role• Experience leading in a matrix environment • Excellent interpersonal and strong communication skills• Demonstrates good oral and written presentation skills• Strong organizational skills, including the ability to prioritize, multi-task, and manage workload to meet specific timeframes and deadlines
