Position Purpose: The Certified Nurse Assistant is responsible for providing various care services to meet the personal needs, support, and health status of participants.

What You Will Do:

  • Provide personal care assistance to maintain participants' independence in activities of daily living, including toileting, showers, eating, and grooming.

  • Assist participants in maintaining physical activity, exercise, and walking.

  • Offer behavioral supervision or support, including reality orientation, medication reminders, dietary restrictions, and emotional reassurance.

  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

Minimum Qualifications:

  • High school diploma or equivalent required.

  • Active certification as a Nurse Assistant in Delaware.

  • Valid driver’s license and use of personal vehicle required for home care assignments.

  • Experience in a long-term care facility or community-based geriatric program preferred.

  • Minimum of one (1) year of documented experience working with a frail or elderly population, or appropriate training from PACE organization upon hiring.

  • Strong conflict management and customer service skills, adaptable to changing work priorities.

  • Proficiency with Microsoft Office suite and electronic mail/calendar systems.

  • Ability to engage in physical tasks such as kneeling, crouching, reaching, lifting, sitting, walking, bending, and lifting up to 20 pounds.
