On Site - Full Time Position

The goal of the Acute Care Manager is to coordinate and manage care to assist patients in meeting their multiple needs, ensure handover and continuity of services, and to decrease fragmentation of care and services. Works collaboratively with members of the

interprofessional acute care, ambulatory and post-acute care management teams to improve communication across the continuum of care.

  • Participates in Performance Projects (A3) to be determined by Manager/Director.

  • Ensures appropriate clinical documentation to support level of care. Analyzes the patient's current clinical status, treatment plan and past medical history to identify potential gaps on physician documentation. Identifies the principal diagnosis and all secondary conditions, co-morbidities, complications and procedures that have been documented in the medical record.

  • Serves as a consultant and educator with medical staff regarding status, placement, payer requirements, and utilization of resources.

  • Assures appropriate sequencing of care/interventions for optimal results and smooth transition along hospital care and continuum.

  • Proactively identifies and removes barriers that impede the progress of care. Coordinates level of care appropriateness with admission status determination.

  • Assures clear admission order. Meets and acts on cases not meeting admission criteria. Anticipates principal diagnosis and procedures.

  • Maintains current knowledge of basic health care economics, trends and reimbursements methodologies and applies knowledge to daily practice. Recognizes situations that require immediately intervention/ Continues to develop professionally and apply knowledge and skills necessary to provide care appropriate to each patient.

Minimum Qualifications

  • A Bachelor's Degree in nursing is preferred with a minimum of 3 years clinical experience.

  • Experience: 3-5 years of direct patient care or case management experience in a similar size, similar scope hospital preferred.

  • Special Training/Skills: Acting independently, must demonstrate an ability to analyze clinical, financial, and social situations, identifying problems and alternative courses of actions, and directing the healthcare team, with a sense of urgency and a drive

Licensure

  • Current license as a Registered Nurse in the State of Florida.

  • BLS Required; ACLS Preferred
