Community Medical Centers of Fresno, CA Project Manager, Strategy and Operations, Community Health Partners in Fresno, California

The Project Manager of Strategy and Operations at Community Health Partners' (CHP) is a subject matter expert in the Project Management Office (PMO) who is responsible for coordinating operational process improvements and strategic growth initiatives with the Community Provider Network (CPN). Functions as a liaison between internal and external groups to facilitate requirement gathering sessions, document requirements and use cases, set expectations and coordinate the delivery of solutions. Provides project coordination and management services and supports business and technical analysis, costs and resource estimates, work plans, documentation, etc. for assigned initiatives. Promotes and adheres to established project management principles and best practices in all aspects of clinical and business operations.


  1. Leads and coordinates various projects by providing unified oversight, ensuring transparency & communication to key stakeholders, and driving successful results.

  2. Conducts appropriate project planning activities with business stakeholders to identify project scope, desired business outcomes, critical success factors, resource requirements, possible implementation methodologies and plans, as well as project risks.

  3. Determines most appropriate implementation toolset and delivery approach specifically designed to meet the needs of the organization and providers. Leverages appropriate data tools to design, modify and populate relational databases.

  4. Collaborates closely with providers and administrative staff to develop a thorough understanding of their programs and assist them in meeting their goals.

  5. Performs appropriate data quality and profiling activities to ensure that information delivered is reliable and consistently high quality.

  6. Identifies, develops and supports implementation of internal process improvement solutions that upholds the network’s goal to enhance clinical and operational performance improvement.

  7. Develops high level understanding of regulations and legislations around hospital mandated reporting on quality and regulatory metrics to support practice and network programs and ensure compliance with state/federal regulations and performance criteria as laid out by CDPH, CMS, TJC and other regulatory bodies.

  8. Performs miscellaneous job related duties as assigned.


• Bachelor's Degree in Business, Computer Science, Engineering, Information Systems, Health Management or related field required

• Master's Degree in one of the aforementioned fields preferred


• 3 years of experience managing projects, supporting operational/business decisions or related required

• Experience with performance improvement practices in healthcare and with data analysis and visualization tools preferred

Licenses and Certifications

• PMP - Project Management Professional Certification preferred
