Parkview North Hospital Ortho Care Navigator - MA in Fort Wayne, Indiana

Summary

Primary role is to assess patient needs in order to perform appropriate interventions to support the patient in the orthopedic service line continuum of care. Works collaboratively with various members of the health care team to provide guidance and support to patient to reduce obstacles of care, and to facilitate a clear path back to baseline. Is a creative problem-solver who serves as knowledgeable resource and coordinator between client, healthcare team, and available community resources. Assists patients with obtaining community assistance by referral and coordination with appropriate resources and scheduling and preparation for appointments. Helps to direct patients to appropriate support while maintaining necessary records and data. Promotes patient education. Builds and maintains relationships with patients and service line providers for purposes of improving quality of care. Understands the uniqueness of the individual and seeks the appropriate resources.

Education

Must be a high school graduate or the equivalent with GED. Must have completed a medical assistant program that meets certification eligibility requirements.

Licensure/Certification

Must be a Certified Medical Assistant (CMA) through American Association of Medical Assistants (AAMA) or Registered Medical Assistant (RMA) through American Medical Technologists (AMT) or Certified Clinical Medical Assistant (CCMA) through National Healthcareer Association (NHA) or National Certified Medical Assistant (NCMA) through National Center for Competency Testing (NCCT) or Clinical Medical Assistant Certification (CMAC) through American Medical Certification Association (AMCA). CPR required

Experience

Medical assistant or physician office experience required. Referral management and scheduling experience a bonus.

Other Qualfications

Must have solid verbal and written communication skills. Knowledge of clinical and clerical operations, service excellence, clinical quality initiatives preferred. Must be familiar with computers, electronic medical record experience preferred. Must be able to lift 25 pounds.
