GENERAL DESCRIPTION

Provides assistance to clients, families, caregivers and office staff with home management, emotional support, and other supportive tasks related to terminal illness and end of life as defined by the Interdisciplinary Group (IDG).

Requirements

ESSENTIAL DUTIES

Adhere to the established hospice care planning providing support services.

Assists with assigned home management tasks.

Provides companionship or relief to caregivers as indicated on the hospice care plan.

Performs other activities as directed by the IDG according to the hospice care plan.

Assists with maintenance of a safe physical environment.

Reports changes to the Volunteer Coordinator or Client Service Manager.

Runs errands, as needed.

Completes daily Activity Records of assistance provided.

Maintains a positive rapport with clients and families, as evidenced by client satisfaction.

Performs day to day administrative duties as assigned.
