The Daily Living Support Specialist at Woodside Prairie is essential in delivering a variety of services to individuals who have co-occurring conditions, such as autism spectrum disorders. The best part of the job is that you will work on a fun and energetic team. The goal is to promote skill development and support community-based living through teaching, coaching, mentoring, and offering assistance tailored to each individual's goals. Our range of services includes community navigation, medication management, assistance with personal daily living skills, monitoring of physical health, and support for overall wellness.

Essential Job Functions:

  • Provides a full range of independent living skills, in and outside of the home, and supportive services promotes the person’s choice and preparation in their day-to-day life

  • Performs and properly documents service delivery on a timely basis.

  • Maintains service level and appropriate contact with people using services and external stakeholders (i.e., funder, service coordinator) to ensure service/treatment plans are followed.

  • Teaches and assists people using supportive and independent daily living skills, including but not limited to, approved nurse delegated duties, personal care, housekeeping and home maintenance, mobility and accessing transportation, interpersonal relationships, safety practices, routine decision making, utilization of community resources, assessing appropriate professional medical services, simple budgeting and consumer transactions, and leisure skills.

  • Teaches, reminds, administers, and monitors medication adherence as directed by a physician.

  • Reports to program management, changes in behavior, functioning, or condition.

  • Serves as a program ambassador by facilitating positive experiences onsite and in the neighboring area, participating in professional networks, and engaging with affiliated organizations; present at internal and external conferences and trainings as opportunities are available.

  • Practices and promotes safety at all times. Demonstrate sound judgement when making safety related decisions particularly in community settings. Report all incidents/accidents according to agency policies and procedures.

  • Performs special assignments and or projects as assigned.

Qualifications

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)

Minimum Required Education: Must have a High School Diploma/GED

Minimum Required Experience: Must have at least 6 months of experience working with individuals who have intellectual and developmental disabilities. The preferred candidate will have professional experience supporting people with Autism Spectrum diagnoses or extended similar personal experience.

Travel outside of office: 40-50% travel in the communities we serve

Required Valid Driver’s License: Valid Wisconsin driver’s license

Required Auto Insurance: vehicle liability insurance in accordance with Agency policy

Knowledge – Skills – Abilities:

  • Must be able to travel via personal vehicle into various communities throughout Milwaukee and the surrounding areas in any weather conditions that don’t present immediate danger.

  • Knowledge of habilitation support for people with various kinds of disabilities.

  • Demonstrated proficiency using a laptop and phone applications. Ability to navigate through a variety of Microsoft programs and ability to learn multiple different computer programs.

  • Must be able to effectively communicate with MCFI Home Care team members, external case managers/care coordinators, people who use services and all other stakeholders in professional manner.

Physical Requirements, Visual Acuity, and Work Conditions:

Physical Requirements : Must be able to push, pull, lift, and move objects weighing up to 50 lbs. and safely transport a client in a wheelchair on a level floor or wheelchair ramp in various weather conditions.

Visual Acuity : The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.

Working Conditions : You should be able to handle stress effectively, especially in situations involving participants with cognitive delays, complex health and mental health issues. You'll also manage multiple tasks, handle noises and interruptions, and work well in a team, all while maintaining a positive attitude.
