Job Description

Position Summary: As the QIDP the Program Coordinator has the decision-making responsibility for the overall day-to-day operation of the Day Habilitation Program. He/she ensures that the program operates in a manner in which is consistent with both the agency and program specific mission statements. The Program Coordinator ensures compliance with all regulations, agency policy and procedures, and operates within budgetary limits. Reports to the Program/Site Director of Day Habilitation.

Essential Duties:

  • Ensures supervision for a team of Supervising Habilitation Specialist; Habilitation Specialist; Assistant Habilitation and Per diem staff

  • Participate in interviews and recommend for hiring of new Supervising, Habilitation, Assistants and Per diem staff

  • Provides direct leadership and training to staff associated with customer satisfaction, program management needs, regulatory compliance standards including OPWDD, OMIG and Agency’s policies and procedures.

  • Ensure development and implementation of program plan, and curriculum development

  • Retrieving, review and have available for audit purposes annual and semi-annual documents including Comprehensive Functional Assessment (CFA), Life Plans, 6 / semiannual, monthly reviews, Level of Care Eligibility Determination etc.

  • QIPD review of documents including but not limited to timely submission of required weekly billing, day Hab plans, monthly summary and all survey required documents.

  • Develop and deliver performance appraisals, manages the progressive disciplinary process including implementing performance improvement plans as needed, and maintaining the necessary records associated with such.

  • Utilizes management tools and databases including the use Electronic Health Record systems to ensure service planning, delivery, referrals and linkages to community-based organizations, follow-up, and collaboration with collateral contacts, while ensuring confidentiality.

  • Required to display proficiency while entering person served data documentation in the electronic data system.

  • Serves as point of contact for crisis intervention for staff and community providers i.e., evaluate and actively solve concerns as needed

  • Collaborates with referral sources including clinical, care providers, transportation agencies etc.

  • Represent the agency in a professional manner at all times.

  • Other responsibilities as deemed necessary by the Program Director of Day Habilitation.

    Other Responsibilities:

  • Develop and ensure implementation of all operation systems including evacuation procedures.

  • Conduct staff meetings on a regular basis.

  • Ensure that the weekly census is completed in an efficient and timely manner.

  • Ensure implementation and participating of appropriate systems for: screening, admissions/discharge, referrals, ISP review, organization, and staffing patterns.

  • Monitor participant’s progress by observing individual and staff performing tasks

  • Participate as a member of committees as assigned.

  • Must be able to physically interact with the program individuals, negotiate the environment independently including assisting in all SCIP interventions, emergency CPR and or first aid techniques and fire drills.

  • Maintain confidentiality at all times.

  • Other duties as requested.

Qualifications

Bachelors Degree required and at least two years supervisory experience preferred. Masters degree in Special Education, Rehabilitation Counseling, Social Work or related field preferred, Demonstrate proficiencies in Electronic Health Records and other: Valid New York State Drivers license for at least 2 years required and access to a vehicle. Excellent communication skills and customer service skills.
