Senior Community Care of Northern Kentucky is looking for a Personal Care Services Manager (LPN) to join our NEW TEAM! If you have a passion for service, affinity for caring for the senior community and have supervisory experience this position is for you!

Salary: $34-$38

Schedule: M-F, occasional weekeds-8 hour shifts

OBJECTIVE: Under the supervision of the Nursing Director, supervises Personal Care Scheduler to
ensure adequate staffing is obtained to meet the needs of participants while remaining at or under budget.
Supervises all direct staff in the Adult Day Health Center and the Home Care departments in conformance
with policies, procedures, and standards and all governing state and federal regulations. Responsibilities
include the planning, coordination, and implementation of the dementia programming for all direct care
staff.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each
essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or
ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform
the essential functions.
1. Bachelor's Degree or equivalent education and experience.
2. Three (3) years' experience in a geriatric health setting including a minimum of two (2) years of
demonstrated successful supervisory experience. Management experience preferred.
3. Certification or equivalent experience demonstrating knowledge of comprehensive dementia care.
4. Proficiency with various business software, preferably Microsoft Office.
5. Must have a valid driver's license, proof of insurance and have means of transportation.
6. Must have medical clearance for communicable diseases and up-to-date immunization before having
direct participant contact.

Skills and Knowledge:
• Knowledge of principles, practices standards and techniques of a Certified Nursing Assistant (C.N.A.).
• Working knowledge of the PACE regulations and state regulations.
• Working knowledge of healthcare and aging networks.
• Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
• Working knowledge of quality improvement and cost containment systems.
• Demonstrated effective leadership and decision making skills.
• Demonstrated strong facilitation and conflict resolution skills.
• Ability to foster and build professional relationships among staff.
• Ability to effectively and efficiently plan, prioritize and follow-up on/delegate responsibilities.
• Effective written and oral communication skills.
• Working knowledge of local health care and geriatric service networks.
