Family Resource Center of Northwest Ohio, Inc. SUD Case Manager in Findlay, Ohio
Who are you? You are a caring professional who has a HSD/GED with previous experience working with SUD population. You have a passion for making a difference in your local community by providing case management services through direct contact with clients, families and essential others in their natural environment. Under the guidance of the Clinical Supervisor you will serve as a client advocate, acting in their best interest at all times. You will attend treatment plan development and review meetings. You will also coordinate and monitor appropriateness of all services, examine and recommend new programs, and provide assistance in crisis situations to stabilize your clients. You have a passion for providing mental health and/or substance abuse services for your clients to stabilize them within the local community. Record client progress, monitor relapse plan and maintain necessary records and files. You will ensure adequate client care, provide consultative services and/or promote the coordination and development of mental health and/or substance abuse services within the community. You will provide support, resources and linkage to services for your clients by maintaining a relationship with other agencies or external service providers as needed. Working in a team based environment is important to you. While working with the treatment team, you will maintain a strengths-based approach as you work with your clients and families to assist with the retention of dignity and the development of positive qualities in the client. - HSD/GED
- A valid driver's license and the ability to maintain auto insurance, as the position requires transporting clients in your personal vehicle throughout the community
- CPR/First Aid Certification or willingness to obtain one
- Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously.
- Willingness to submit background check and fingerprinting
- An active CDCA (Chemical Dependency Counseling Assistant) certification
Preferably.
- Bachelor's degree in social services related field
