**THIS OPPORTUNITY WILL BE IN CLIENTS HOME WHO RESIDE WITHIN THE FOLLOWING COUNTIES: AMELIA COUNTY, BUCKINGHAM COUNTY, CHARLOTTE COUNTY, CUMBERLAND COUNTY, LUNENBURG COUNTY, PRINCE EDWARD COUNTY, AND NOTTOWAY COUNTY.

EMPLOYEE WILL NEED TO HAVE RELIABLE TRANSPORTATION OR ACCESS TO PROVIDE SERVICES WITHIN THE HOMES OF OUR CLIENTS.**

DUTIES AND RESPONSIBILITIES

Teach/perform meal planning and preparation, routine housekeeping activities such as making beds, dusting, vacuuming, washing floors, cleaning kitchen and bathroom, and laundry.

Assist with a written, special diet plan and reinforcement of diet maintenance, which is under the direction of a Physician and as identified in the care plan.

Provide companionship, friendship, and emotional support.

Assist clients with communication by writing or typing correspondence for them or researching information for them.

Participate in the care plan process by providing input and making suggestions.

Ensure service is delivered is accordance with all relevant policies, procedures and practice.

Monitor supplies and resources.

Evaluate the program and make recommendations to it, as indicated.

Follow the written care plan.

Carry out duties as assigned by the Supervisor.

Observe the clients functioning and report to the Supervisor

Complete and maintain records of daily activities, observations, and direct hours of service.

Attend orientation, in-service training sessions and staff meetings.

Develop and maintain constructive and cooperative working relationships with others.

MINIMUM EDUCATION AND EXPERIENCE

Employee must have a current driver's license and proof of proper vehicle insurance coverage. Preferred high school diploma.

PHYSICAL DEMANDS AND WORK ENVIRONMENT

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger handle, feel objects, tools or controls; reach with hands and arms; climb stairs; balance, stoop, kneel, crouch or crawl; talk or hear; or smell. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee is generally will be exposed to weather conditions. The noise level in the work environment is usually moderate. Daily travel may be required.
