Presbyterian Homes Social Worker in Evanston, Illinois

Westminster Place is looking for an LSW/LCSW Social Worker to join our team.

Salary Range based on experience: $57,135 - $68,455 annually.

    Main Function:

    To formulate and implement plans for bringing together the needs and requirements of intermediate and skilled care residents and families with the resources and services of Westminster Place, to support and assist their continuing activities in the Westminster Place community as well as the larger community setting.

    To assess and coordinate the needs and requirements of older adults with the services offered by Presbyterian Homes in support of and assistance with maximizing continued activities within the Westminster place community and the community at large.

    Reports To : Healthcare Administrator

    Essential Duties:

  • Provide counseling and support to residents, families, and friends of residents in individual settings.

  • Function as an integral member of the multidisciplinary team; collaborate and provide clinical consultation as needed.

  • Assist residents in obtaining services that enhance their ability to function at the highest possible level of independence.

  • Maintain and protect the confidentiality of resident information.

  • Foster effective communication and teamwork among residents, families, and staff.

  • Develop and present educational and psychologically supportive programs and for residents.

  • Provide appropriate psychosocial assessments and documentation in resident records in compliance with federal and state guidelines.

  • Maintain communication with families of residents to inform them of any special requirements that a resident may need.

  • Inform individuals of their rights and responsibilities as residents of Presbyterian Homes and serve as resident advocate.

  • Participate in interdisciplinary committees focusing on the resident’s health care needs.

  • Develop and maintain community resource information lists.

  • Promote the Presbyterian Homes’ vision, mission, and values.

  • Participate in professional seminars and workshops to maintain LCSW licensure.

    Education: Master’s Degree in Social Work and LSQ/LCW license by the sate of Illinois.

    Experience: A minimum of two years post graduate social work. Geriatric experience desired.

    Qualifications:

  • Possess a Social Work License in accordance with the1989 Social Work Practice Act of Illinois, Revised status.

  • Counseling skills, communication skills. Capable of independent judgment, initiative, and ingenuity. Ability to become an active participant in the inter­ disciplinary health care team.

  • Have a proactive personality with an emphasis on teamwork and flexibility.

  • Must be able to work in an unstructured environment, handle several tasks simultaneously, problem solve and respond quickly and effectively.

  • Possess experience facilitating groups.

  • Ability to maintain confidentiality.

  • Desire and ability to work with older adults. Computer literacy.

  • Contacts Staff of all departments and levels, residents, families, community professionals, ministers, physicians, volunteers.
