15.00-17.50 per hour Biweekly | Closes 12-Jun-2024 (PST) | 3690 West 1st Avenue, Eugene, OR, USA | Hourly | Adult & Community Services | Part Time | All: PTO, Holiday Pay, Gym Membership, EAP, Paid Training, Reimbursements. Full Time: Medical, Dental, Vision, 401k.

POSITION SUMMARY : A Supported Living Provider is a life helper to individuals who possess a variety of abilities. Our clients need some assistance to engage in community activities such as shopping, going on outings or getting a workout at the gym. A Supported Living Provider will also assist individuals in their homes, mentoring basic life skills such as keeping a clean house, managing finances and dealing with situations that enter people's daily lives. Providing this type of human service is very rewarding knowing you are truly making a difference in the quality of someone's life. This position is less than 30 hours per week.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Transport clients and assist with errands and appointments such as grocery shopping, banking and medical appointments

  • Assist with order and pick up of prescriptions, understanding medication instructions, filling med boxes and scheduling medication reminders

  • Assist with accessing community resources such as food, housing and family services

  • Assist as needed with applications for various services

  • Provide support for community inclusion including hobbies, exercise, art and music classes, social groups, outings, and other preferred activities

  • Provide hands-on assistance and reminders with household chores and responsibilities, such as cooking, cleaning and life organization

  • Assist with activities of daily living such as hygiene and personal care, offering physical support as needed (transfer, ambulation, or standby assistance)

OTHER DUTIES AND RESPONSIBILITIES:

  • Maintain all necessary client records and record daily data entries into appropriate record keeping systems

  • Cross training in other areas including Job Coaching

MINIMUM QUALIFICATIONS AND EXPERIENCE :

  • High School Diploma or equivalent

  • Dependable transportation; valid Oregon driver's license with driving record sufficient to qualify to drive agency vehicles

  • Ability to pass an Oregon Department of Human Services background check

  • Experience working with adults with intellectual/developmental disabilities preferred
