PeaceHealth is seeking a Nurse Practitioner for At Home Primary Care based in the Eugene community.

We are seeking a Nurse Practitioner who has a minimum of 2 years' experience working with patients in the nursing home environment as well as doing home visits with frail patients.

At-Home Primary Care sees patient where they live-whether a house, apartment, or assisted living unit. (This does not include skilled nursing facilities.)

Purpose

At-Home Primary care aims to improve access to care and lower the total cost of care for patients enrolled in this home-based primary care program in the Greater Eugene-Springfield area. The program uses and interdisciplinary team approach to:

  • Partner with patients and their families

  • Reduce emergency department visits and inpatient admissions.

  • Improve overall health and patient experience.

Services

  • Transitional Care Management (TCM)- for patients who are discharged from inpatient care or a skilled nursing facility.

  • Co-management – For patients who are temporarily unable to go to a clinic. At-Home Primary Care will co-manage care with the patient’s PCP. When the patient can return to in-person visits, the patient’s PCP resumes full care.

  • Primary Care-For Patients who are not expected to regain the ability to go to clinics for in-person visits. At-Home Primary Care Will serve as sole PCP.
