JOB REQUIREMENTS: I. Provides Case Management Services A. Recruitment and pre-screening of potential applicants Maintains regular contact with appropriate area referral sources. Interviews applicants, including self-referrals to determine eligibility. Collects documentation to verify eligibility. Assists in review of completed eligibility determination forms. Maintains pool of eligible applicants necessary to meet targeted program criteria by reaching out to county referral resources as necessary. IE: Senior Centers, Food Pantries, Housing authorities, ADRC\'s, B. Provide orientation on available services, career information, and job-seeking skills C. Provide/coordinate career assessment to assist participants in determining interests, aptitudes, abilities, limitations and training needs, and/or refers to additional assessment as needed. D. Assists individuals in developing employment/training plans and service strategies to address assessed needs Identifies appropriate training activities and/or job placement strategies based on assessment. Identifies needed supportive services and coordinates the provision of such services with appropriate providers. Provides ongoing advocacy to individuals enrolled in services. Keeps accurate and timely case notes in SCSEP case note database Enter enrollment data into DOL database E. Assists participants in the implementation of individualized training/employment plans. F. Provides participant follow-up to assess progress and assist in retention G.
