Description LSS of WI and Upper MI is currently searching for a Care Coordinator for our YES program in Eau Claire, WI! YES provides wraparound services to at-risk children and their families, focusing on children and families who have struggled with traditional service provision and are either at-risk for out of home placement or who are working toward reunification. YES provides intensive case coordination and individualized community-based services. Services focus on the identified child\'s well-being and progress, but incorporate a family members and supports, acknowledging the important and crucial roles they play in the lives of children and juveniles. This is a full time, 40 hour a week, benefit eligible role. Under direction, performs work involving the assessment, service planning, and implementation of the service plan for assigned individuals. Performs other duties as required. Assess client\'s needs utilizing the required assessment tool(s) (e.g. functional screen). Re-assess, as needed, and at least annually. Working alongside the client, their family, his/her supports, and other service providers, develop and implement a comprehensive and individualized service plan.
