KPH Healthcare Services, Inc. Medication Therapy Management Technician in East Syracuse, New York

Overview

Scope of Responsibilities: Assist with the day-to-day operations of the centralized medication therapy management/clinical team at Kinney Drugs. Works under direct supervision and follows standard procedures to accomplish assigned tasks. Provides centralized support to the retail pharmacy store teams to achieve pharmacy service goals related to medication therapy management and performance-based programs.

Job Summary : Assist with medication therapy management (MTM) case preparation, scheduling of comprehensive medication reviews, complete technician driven MTM targeted medication interventions and outreach to non-adherent patients to offer and complete enrollment in automatic refill programs and to ensure annual adherence.

Job Duties:

  • Centrally prepare CMR cases on various platforms (for example, OutcomesMTM, MedMonitor) by reconciling allergy and medication list from the platforms with prescription history in pharmacy dispensing platform.

  • Running daily reports from various platforms to communicate and prioritize new patient cases to the centralized pharmacist team.

  • Scheduling of comprehensive medication reviews with our centralized pharmacists.

  • Complete technician driven targeted interventions (Pharmacy TIPs, technician driven TIPs, Adherence Reports).

  • Utilize outlier reporting systems to run, filter and merge adherence-based reports to gather data and relay deadlines to centralized pharmacists.

  • Contact non-adherent patients to offer ReadyScripts and improve patient adherence scores.

  • Attend various meetings in regard to MTM performance and various clinical programs.

  • Maintain confidentiality regarding all patient information.

  • Must keep current with all State and Federal requirements as related to conditions of employment.

  • Responsible for completing all mandatory and regulatory training programs

  • Perform other duties as assigned.

Qualifications

Educational Requirements:

  • Minimum: High School Diploma (or currently enrolled) or GED

  • Preferred: AS Degree or Higher

Experience:

  • Preferred hospital or retail pharmacy operations

Required Licensing:

  • Vermont Technician license is required or willing to become licensed within 3 months of starting this position.

Required Certification:

  • National Pharmacy Technician Certification through PTCB, or another accrediting agency, in good standing

  • PTCB MTM Certificate Program

  • Special Conditions of Employment:

  • Any and all additional eligibility requirements based on the specific position

  • Drug Test

  • Initial and continuous exclusion and sanction/disciplinary monitoring

Compensation

$16.11-20.16 per hour.

The final offer will be determined after careful consideration of multiple factors such as relevant skills, years of experience,and education.
