Community Partners Job & Life Skills Coach in Dover, New Hampshire

The heart of our non-profit Agency’s mission is empowering the people we serve to live a full life in their community. PDMS (Participant Directed and Managed Services) provides assistance to clients age 21 and older who participate in our Self-Directed Services program. Our supportive team is looking for Multiple Candidates to fill the role of FME’s (Family Managed Employees) who are self-starters, have flexible hours, and are excited to make a positive difference in others’ lives. * 21-year-old man on the autism spectrum. 30 hour position working with a young man out of his home in Rochester NH, specific hours/schedule to be determined upon interview. Supports are needed to engage in both home and community based activities, with a focus on building comfort level and willingness to engage. The ideal candidate will have an interest in DD, anime, gaming, WWE, role playing and tabletop games, and the ability to redirect and assist individual in managing anxiety. This candidate must be compassionate and able to provide reassurance and encouragement, must be comfortable with dogs, as this is a pet friendly home. This staff would be a family managed employee, working directly with the family and the area agency. * Man in his thirties in the Dover area. The schedule for this position will vary and will include evenings, weekends and when staff are on vacation. Average need will be 6-8 hours per week. Payment for hours worked would be paid directly by the family and not through Community Partners payroll. This position involves providing support and assistance with community access, going to the gym, participating in aquatic therapy, assisting with personal care and activities of daily living (ADL's) and accessing recreational opportunities in the community. * 28 year-old woman in the Dover area. Seeking a friendly, self-motivated and dependable individual to provide community based support. The position will involve in skill building around money management and literacy skills, volunteering in the community, accessing physical health activities and increasing vocational skills. Client's interests include working with animals, skiing, track field, basketball and reading. 35 hours per week. * 25 year-old woman in Strafford County. Seeking a friendly, motivated, dependable and responsible individual to support this young woman build home living and life skills and increase independence. The ideal candidate will have the ability to model good work ethics and be motivated to identify new opportunities and activities. 35 hours per week. * Woman in mid-twenties in the Somersworth area. Seeking a provider to assist with developing skills related to home living (cleaning and organizing apartment), accessing the community, support with grocery shopping, banking/money management, planning a weekly schedule and attending medical appointments. 5 hours per week. Schedule to be determined. * 28-year-old woman in Dover, NH. This position will involve providing support and assistance with activities such as: building money management and literacy skills, volunteering in the community, accessing physical health activities, and increasing vocational skills. The client’s interests include working with animals, skiing, track field, basketball, and reading. The ideal candidate will be someone who has experience working with individuals with disabilities and who is energetic, observant, and has a great sense of humor. 35 hours per week; hours/days to be discussed upon interview. HS Diploma or Equivalent required. Experience with behavioral health and developmental services is preferred, but not required. Transportation is a requirement of this position, so a valid driver’s license, clean driving record and driver’s insurance is required. Community Partners, Strafford County’s premier behavioral health developmental services provider, links people to the long-term supports and services they need to a live a full life in this community. In the Self-Directed Services program, families in Strafford County manage their own services (including staffing) for an adult family member who has a developmental disability or a traumatic brain injury. The employee will work out of the family home and the family is the Managing Supervisor with Community Partners as the Employer of Record.
