Genesis Healthcare Clinical Hospital Liaison in Dover, Delaware

This positions supports the referral process for our centers in the Seaford, DE market.

Blend your clinical and marketing skills to build hospital partnerships, manage patient safety and outcomes, and manage patient cases as a Nurse Liaison. In this role, you will position Genesis as a strategic partner with select referring hospitals by concentrating interactions on patient cases and improved outcomes. Encourage meaningful collaboration with hospital-based case managers on risk factors and care plans and identify new opportunities for growth partnering is also part of responsibilities. You'll educate hospital staff, including ER physicians, of Genesis nursing centers' clinical capabilities and appropriate patients for return to skilled nursing facility as a viable alternative to acute care transition. Some of the additional responsibilities are as follows:

Reengage with re-hospitalized Genesis patients to ensure their return to a Genesis facility when appropriate. Connect with hospital staff to effectively document recent six months' emergency department utilization by patients, signs/symptoms that prompted emergency department visits, primary diagnosis and comorbidities, and follow-up orders for completion.

Facilitate regular meetings with hospital staff to review patient cases with high frequency of rehospitalizations, conduct root cause analysis and develop/implement performance improvement plans.

Identify risk markers and effectively communicate to center-based staff and Transitional Care Nurse for patient transition from a Genesis facility to home.

Maintain Genesis electronic medical record Point Click Care (PCC) and Allscripts/Curaspan on patient population.

Qualifications:

RN License. Five to seven years of recent acute care, post-acute care or long-term care experience is required. Experience in business/development preferred. Prior experience in utilization review or discharge planning preferred.
