Job Requirements:

Provides supervision and medical oversight to ensure that clients are being served effectively and safely and supervises all client care and services.

  • Responds to calls within 30 minutes

  • Conducts supervisory visit at least once every 60 days

  • Ensures services are  delivered as indicated on the client’s care plan

  • Conducts initial evaluation of clients

  • Assesses, evaluates, and monitors clients’ needs, current status, and environment

  • Implements appropriate interventions

    Documents accurately, appropriately, and in a timely manner

  • Prepares progress notes

  • Documents, signs, and dates supervisory visits notes

  • Documents evaluation of client’s health status and needs

  • Documents interventions and outcome of those interventions

  • Completes client’s discharge summary when indicated

    Maintains clients’ clinical records in accordance with acceptable professional standards and guidelines

  • Conduct audit on clients’ record monthly (end of month)

  • Ensures clients’ records are readily accessible and organized

  • Ensures confidentiality of clients records

  • Instructs staff on proper charting protocols

  • Reviews, signs, and dates all entries made by non-licensed staff

  • Reviews Level of Care and Placement Instrument at least  

    Maintains ongoing and effective communication with Care Coordinators, clients, caregivers, staff, physicians, and all other relevant parties

  • Communicates problem approaches, changes to client care plans, and other pertinent information to staff

  • Communicates recommendations for changes in the clients’ care to Care Coordinator

  • Obtains Care Coordinator’s approval prior to making changes in client’s services

  • Reports problems regarding clients’ health services to Care Coordinator in timely manner

  • Submits notification of start of care to Care Coordinator within 2-3 days

    Monitors client’s medications

  • Is aware of all client’s prescription and over-the-counter medications

  • Reports concerns about client’s medications to Care Coordinator and client’s physician

  • Instructs staff on level of assistance clients needs with medications

  • Reviews, signs, and dates client’s medication administration record at least every 60 days

  • Provides written instructions to staff regarding signs and symptoms of potential drug reactions

  • Records any client reactions to medications in the client’s clinical record

    Initiates, reviews, and revised Client’s Care Plan

  • Coordinate supervisory visit every 60 days

  • Initiates client’s care plan within 60 days of initial evaluation of client

  • Reviews, signs, and dates client’s care plan at least  every 60 days

  • Ensures client’s care plan identifies specific physical, mental, and social health problems/needs

  • Lists personal care tasks  needed on client’s care plan

  • Re-writes the client’s care plan at least once per year

    Coordinates and participates in learning opportunities for staff and clients

  • Conduct interview for potential LPN and RN with General Manager

  • Schedules, coordinates, and presents monthly in-service training to staff

  • Ensures appropriate health related topics are presented to staff

  • Instructs client and other care-givers about clients’ health needs

    Assists with continuous quality improvement program

  • Evaluates quality of client care during face-to-face evaluation with clients

  • Conducts monthly compliance visits

  • Reviews policies and procedures at least twice per year

  • Recommends revisions to policies and procedures as needed

  • Reviews at least 10% of client clinical records each month

  • Ensures staff compliance with infection control policies and procedures

  • Ensures staff compliance to fire and safety policies and procedures

  • Presents neat, professional appearance

  • Demonstrates positive attitude

  • Promotes teamwork
