Job Summary

Under general supervision, works as full member of the MICU multi-professional critical care team. Contributes to excellence in patient care, research, teaching, and provides leadership to the organization. Functions as a medical intensivist under the supervision of the MICU Director or his designee. Collaborates with resident physicians in internal medicine and manages the critical care aspects of the assigned patient's intensive care stay in the MICU and progressive care unit (PCU) from admission to transfer or discharge. Demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence including expertise in central line insertion, airway intubation, arterial cannulation, lumbar puncture and si This position includes weekday, weekend, and evening coverage but may expand to 24/7 coverage in the future. The critical care APP will additionally have responsibility for quality and performance metrics and improvements in conjunction with the MICU leadership and will be required to contribute to educational and research activities in the MICU/PCU.

Essential Functions:

Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients. Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Coordinates daily rounds with the Attending and Intensivist physician teams, presents 24 hour patient report, and collaborates in plan of care. May serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority Diagnoses complications and orders appropriate interventions. Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address immediate critical care needs. Utilizes evidenced based practice guidelines, pathways or protocols in an individualized, dynamic plan of care that can be applied across the continuum. Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes. Prescriptive authority for prescribing, ordering, and or administering drugs; including (but not limited to) schedule II, III, IV, and V drugs and necessary medical devices. Manages further diagnostic tests through ordering, interpretation, performance, and supervision. May perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals. Assesses educational needs of the patient-care staff and provide one-on-one as well as formal education as needed. Maintains ongoing communication and collaboration with interdisciplinary health care team. Assesses educational needs of the patients / families and provide culturally-sensitive patient/family educati n interventions as indicated. Use non-technical language when educating patients and families. Document all pre and post-care education in patients' medical record. Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient continuum of care. Implements and evaluates appropriate educational programs for DHMC MICU patient care staff and other healthcare providers as appropriate. Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable. (H&P, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries in medical record per specific patient unit or service, death summary) Documentation is timely, meets critical care compliance standards and captures patient acuity.
