Facility:Work From Home - OhioDepartment:HIM - OperationsSchedule:Full timeHours:40Job Details:Responsible and accountable for timely and accurate release of patient health information, proper documentation and tracking of requests for information, and providing personalized customer service to all requestors. This individual releases information according to hospital policies and procedures and state and federal statutes and processes the records for copying and mailing. Uploads electronic records to client portals or burns information to encrypted CD. Assists internal customers with their release of information processes, utilizing hospital policies and best practices. Advises internal customers regarding HIPAA concerns related to release of information. This is a hybrid work location.

Department Specific Job Details:

Weighting %1. Release of Information 30 Understands and follows departmental and legal guidelines for release of patient information. Conveys accurate information to requestor regarding release of information procedures. Handles all requests for patient information, including stat requests. Requests off-site charts needed for release of information to be scanned on demand via Filebridge chart tracking software.  Is knowledgeable of federal and state legislation regulating the release of medical information. Assists with answering and handling incoming phone call requests and walk-in customers with questions pertaining to release of information. Assists internal customers with their release of information processes utilizing hospital and departmental policies. Advise internal customers regarding HIPAA concerns with their departmental release of information.
  1. Processing Requests30 Requests for patient information are processed within 7-10 business days of receipt.  Requests for patient information are processed utilizing Epic Releases module.  Reviews requests for patient information to ensure a valid authorization has been received for the release of information. Stat requests are processed upon receipt. All requests are processed according to departmental procedures. Variances related to release of information are reviewed with HIM Manager or Director prior to release of information. Follows up with departments and physicians for missing and/or incomplete documentation needed to fufill request. Screens records requests for potential risk management cases and forwards to HIM Manager when appropriate. Prepares charts for copying using standard guidelines. Copies charts/prints reports from electronic medical record and prepares copies for delivery based upon request.  Accepts and posts payments accurately for billable records requests. Balances payments received and prepares bank deposit daily with 100% accuracy. Uploads required information to client portals or creates encrypted cd upon request. 3. Release of Information Database20 The release of information database is accurately maintained.  Requests are logged into the database within 24 hours of receipt. Database is reviewed periodically for errors and corrected to show accurate pending reports. Communicates regularly with HIM Manager regarding any issues regarding payment processing. 4. Subpoenas, Court Orders15 Determine validity of subpoenas and court orders, providing the appropriate responses to the attorneys and courts. Subpoenas and court orders are responded to before the required date with an accuracy of 100%. Attorneys are contacted upon receipt of the subpoena to ensure the acceptance of certified copies of records in lieu of a court appearance. Attorneys are also notified that a release of information authorization or court order is required when a subpoena is received. 5. Back-up Coverage5 Provides back-up coverage for the HIM Department Receptionist.. Loose sheet scanning, performed at an accuracy rate of 98% or higher.  Performs quality analysis of scanned documents.
